Fourteen of those years, he admits, he didn’t know the root of a problem some of his patients were facing.
“For many years when a patient would call in and say ‘Dr. Liss, this drug is making me sick,‘“ Liss said. “I’d say, Mrs. Jones, you’ve been on the same drug for five years, why would you think it’s the drug?“ As Liss discovered, it’s not so much the drug as it is the body’s interaction with it, the very art of medicine.
“What I didn’t recognize and what a lot of people don’t recognize is the definition of what equivalent is,“ Liss said about the concept of generic equivalents. What he means is that the FDA allows a range of 80 to 125 percent bio-equivalence for generic drugs. This means that if you are accustomed to a brand name drug and switch over to a generic, your body might not be getting enough (or too much) of the medicine you need.
Liss emphasizes that he’s not against generics. It’s a good medicine, he says, just not the “same” medicine. Same amount, rather. And with too much or too little of a good thing, Liss has seen disease relapse among his patients or side effects as a result.
The factory from which the generic drug came can also influence how it may interact with your body’s chemistry. If your pharmacist buys from one state one month and another the next, the 80 to 125 percent range can complicate your health. Liss recommends becoming “best friends” with your pharmacist. Know when there’s been a change in the factories he or she uses. If your insurance favors a generic, discuss that change with your doctor. Request a prior authorization before filling a prescription to facilitate switching back should you have a relapse or experience side effects. You may also request that your doctor write a letter to your insurance company to legitimize your need to go back to a brand name drug.
