BACKGROUND: A recent survey of STD clinic patients found Viagra use among men who have sex with men (MSM) at 32% and significantly higher among HIV-positive MSM. Similarly, a community-based survey of MSM found Viagra use to be 33%. Because Viagra may be prescribed for erectile dysfunction resulting from AIDS medications or depression among persons with AIDS, we reviewed medical records of AIDS cases in San Francisco in order to characterize Viagra use and assess its potential role in the risk of HIV transmission. METHODS: We ascertained Viagra use through medical chart review of persons living with AIDS in San Francisco in 2001 and characterized its use with respect to demographic and behavioral risk characteristics, and use of highly active antiretroviral therapy (HAART). Logistic regression was used to determine factors associated with Viagra use. RESULTS: Of 4,394 medical records of AIDS cases examined, one female (<1%) and 168 males (4%) had ever used Viagra. Among 4,113 male AIDS cases, Viagra use was significantly associated with older age (odds ratio OR=1.03, confidence interval CI=1.01,1.05) and use of HAART (OR=4.29, CI=2.59,7.12). CONCLUSIONS: Viagra use was less frequent in our study than suggested by previous surveys. Physician prescription of Viagra to persons with AIDS is more frequent among older men in whom erectile dysfunction is more common. Increased use of Viagra among persons receiving HAART may reflect increased sexual activity among persons with AIDS with improved physical function. The finding points to the need for HIV care providers to address prevention issues in their clinic population. Additional studies to assess recreational use of Viagra outside of primary care settings should be conducted to more completely ascertain the potential risk of Viagra use and HIV transmission.
